Trading Hours

This form should be filled out and submitted in advance when a consultant knows that he/she will be unable to work an assigned shift(s). To ensure coverage of said shift(s), the consultant covering the hours must be the one to fill out and submit this form. In cases when you cannot find someone to cover your shift(s), you must contact one of the coordinators at 859-622-7375.
